INVITATION FOR TENDER

Federal Neuro-Psychiatric Hospital Yaba, under the supervision of the Federal Ministry of Health is in the process of executing the Capital projects in the 2019 fiscal year. In line with the Public Procurement Act 2007, the Hospital hereby invite competent contractors/suppliers with relevant experience and good track records to submit technical and financial bid for the under listed projects.

PROCUREMENT OF MEDICAL EQUIPMENT FOR ADL ROOM (FSOT OSHODI)

 

1. ELIGIBILITY CRITERIA
a. Evidence of Registration on the National Database of Federal Contractors/Consultant and service Providers issued by the Bureau of Public Procurement (BPP).
b. Company profile and organizational structure including names, resumes and telephone numbers of key personnel.
c. Verifiable list of previous /similar major contracts carried out in the last five years particularly in health institutions and in government organization.
d. Company audited account for the last three (3) years (2016, 2017, and 2018).
e. Evidence of incorporation with corporate affairs Commission (CAC) and Tax Clearance certificate for the last three years (3)
f. Evidence of VAT Registration and VAT Remittances.
g. Evidence of registration with National Social insurance Trust Fund.
h. Evidence of Registration with National Pension Commission.
i. Evidence of Contribution to Industrial Training Fund.
j. Evidence of payment of =N=10,000.00 non-refundable processing fee to the Account department of the hospital.
k. Sworn affidavit disclosure:
I. That the company is not in receivership, insolvency or bankrupt.
II. That the company does not have a director who has ever been convicted in any court in Nigeria or any other country for criminal offences in relation to fraud or financial impropriety.
III. That no officer of the FNPHY or BPP is a former or present Director, shareholder or has any pecuniary interest in the bidding.

2. COLLECTION OF DOCUMENTS
The tender documents are to be collected from the Procurement Office, with a Tender fee of =N=10,000 receipted from finance department

3. SUBMISSION OF BID DOCUMENT
Bid documents must be submitted in two (2) separate sealed envelopes; one each for the Technical and Financial Bids and clearly marked Technical bid or financial bid at the top right hand corner. All unmarked bids will be rejected.
All bids shall be delivered into the appropriate Bid Boxes at the Office of the Head of Administration Federal Neuro-Psychiatric Hospital Yaba, on or before 12.00 noon on the closing date.
All sealed envelopes must have the company’s Name, Address, and Contact Phone Numbers clearly written at the second side of the envelope and addressed to:

4. OPENING OF BID DOCUMENTS
Tender envelopes will be opened at the expiration of the six (6) weeks i.e. 21st November, 2019 in the presence of the companies’ representatives who choose to attend in person at the Boardroom of the Federal Neuro-Psychiatric Hospital, Yaba, interested members of the public are hereby invited.

YABA GETS SUBSTANTIVE MEDICAL DIRECTOR

The Acting President, Federal Republic of Nigeria Prof. Yemi Osinbajo, GCON has approved the appointment of Dr. Oluwayemi Cecilia Ogun as the substantive Medical Director, Federal Neuro Psychiatric Hospital, Yaba Lagos for a term of Four (4) years with effect from 1st of June, 2017

The Medical Director Dr. O.C. Ogun while addressing a cross section of staff at an interactive session on 6th July, 2017 said that her tenure will be focused on Patient care and Staff Welfare.

She thanked the President and Commander in Chief of the Armed Forces, President Mohammadu Buhari for her appointment.

Dr. Ogun started her Career in Psychiatry when she joined the Residency Training Programme of the Psychiatric Hospital Yaba, Lagos in 1990 and completed the programme in May 1998 when she became a Consultant Psychiatrist.

Dr. O.C. Ogun was appointed as a Consultant Psychiatrist on 28th April, 1999 and over the years, she rose to the post of Consultant Psychiatrist Special Grade I.

She was the Head of Clinical Services from March 2010 to March 2014 and was Acting Medical Director of our great hospital between November 2010 and August, 2011.

Until her appointment she was the Coordinator of the Child and Adolescent Mental Health Services in the hospital.

The Medical Director thanked the former Acting Medical Director, Dr. R.A. Adebayo for holding forth during the transition period.

She thereafter, solicited for the support of all members of staff to her Administration.
